Care Coordination Learning Collaborative 

CYSHCN Cares 2 is a Care Coordination learning collaborative of the Mississippi State Department of Health, Children and Youth with Special Health Care Needs (CYSHCN) Program. This collective learning and sharing opportunity is designed to help participating organizations improve care coordination for children and youth with special health care needs.  According to the Maternal and Child Health Bureau, CYSHCN are defined as children from birth to age 21 who have or are at increased risk for a chronic physical, developmental, behavioral, or emotional condition and who also require health and related services of a type or amount beyond that required by children generally. 

CYSHCN Cares 2 offers training in electronic health record utilization/analysis, quality improvement, and family engagement by focusing on communication and coordination across systems of care.  The goals and measures of this initiative are evidence-based and congruent with national measurement systems in the health industry.
